Hamilton Family Health Team Helping Residents Connect with Primary Care
The Hamilton Family Health Team (HFHT) is working to connect residents across the Greater Hamilton area with primary care providers and is encouraging community partners to help share this important message.
Currently, approximately 60,000 Hamilton residents do not have access to a family doctor. HFHT’s Attachment Team is actively supporting individuals who are looking to connect with a primary care physician that meets their needs.
Services are available to residents living in Hamilton, Haldimand, and Niagara North-West. The Attachment Team reaches community members through outreach efforts at locations such as food banks, libraries, the YMCA, Mission Services, and hospital programs. Support is also available through in-office services, email inquiries, and a dedicated phone line.
If you or someone you know does not have a family doctor, help is available to get connected to primary care.
